The Utilization of Marginal Donors for Cardiac Re-Transplantation

PurposeThe global shortage of available donor organs and the poor clinical outcomes re-transplant patients raise ethical economic concerns. In this study, we examined difference in post-transplant survival (re-HTx) with without use marginal hearts.MethodsThe United Network Organ Sharing database was retrospectively queried from January 2000 to March 2020 identify adult (17 > years) underwent re-HTx. Recipients were divided into early (≦6 months after previous transplant)and late (>6 transplant) groups based on timing re-transplantation. Demographic data compared donors each group. Marginal those who met at least one following characteristics: age greater than 55 years, cocaine or other intravenous drug use, hepatitis C positive, ejection fraction less 0.45, donor: recipient weight 0.8.ResultsThere 111 1133 re-transplantation.10-year mortality significantly higher transplantation (Early 61.0% vs. Late 44.9%, log-rank P<0.001).
